Referral Stream™ offers numerous queries that expedite referral office operations. The Referral Stream™ CRM reports allow management to review the quality of the hospital system's relationship with referring providers, and review the work of individual relationship managers or recruiters. The management reporting capabilities present metrics and other data that measure the success of the ambulatory services program, show clinical utilization, no-shows, and other KPIs needed for program administration and decision-making.

Functional Queries - for Referral Operations

The Referral Stream™ functional queries for referral operations streamline the workflow for receiving, reviewing, approving, scheduling patients, and returning the consultation fulfillment information to the initiating provider. Different search views quickly identify referrals awaiting review, scheduling, follow-up, or closeout actions. Numerous ”click-through” hyperlinks enable users to easily update referral status without having to access the actual referral form. Here are some highlights of the referral operations functional queries:

• View current and past referral activity by hospital facility or clinic, service group, service/procedure, patient identifier(s) phone number, insurance, referral status, practice, provider, referral type, provider’s credentialing category , date or date range of referral creation or appointment.
• Drilldown to events occurring on the day of the query or during a date range.

• Locate referrals by facility, service group, service/procedure, point of service, referral status, and date or date range of the referral or the appointment.
• The query result shows the last status update, and enables users to quickly update the status and send “no-show” letters to providers simply by clicking on a link, avoiding the need to access the actual referral paperwork.

External Provider Search

• Search by provider subtype (physician, dentist, chiropractor, etc.), specialty, gender, language, credentialing category, city, zip code, and staff recruiter/community outreach person to determine the provider’s practice or practices.
• Hyperlink from the search results directly to the provider’s profile in order to update.

Practice Query

• Get details on the practice by entering a practice name or just a fax number, zip code, or practice type (private practice, free-standing facility, or internal to the hospital system).
• See the associated providers and if the practice is their primary or additional practice.

Chart Requests

• A functional tool that enables referral office personnel to generate chart request notices and notices to patient registration to facilitate patient navigation.
• Chart requests ensure that patient records, where not fully migrated to an electronic format, are, delivered to the correct service area. Registration notices are an excellent way to initiate good customer service, enabling the reception desk personnel to welcome patients and direct them to their appropriate point of service.

Referral Fax History – Internal Senders Only

• This is a report from the fax server that lists faxes sent to external providers.
• Links are presented to access a copy of the document sent.
• The fax server reports on the status of the transmission.
• Used for audit and compliance trails.

Pre-Surgical Evaluations - View Sample >>

• This is an operational and clinical report showing patients whose required pre-surgical evaluations or clearances have not been submitted by their PCPs. The trigger is a due date field in the referral form.
